1.备齐用物携至患者床旁,核对患者,请无关人员离开;关闭门窗,用屏风遮挡,询问患者是否排尿。

2.协助患者取左侧卧位,双膝屈曲,脱裤至膝部,移臀部靠近床沿,将垫巾和治疗巾垫于臀下,弯盘置臀旁,盖好盖被。

3.挂灌肠袋于输液架上,液面距肛门40-60cm,排尽管内空气,用血管钳夹紧肛管。

4.用石蜡油棉签润滑肛管前端。

5.左手分开臀部,暴露肛门,右手将肛管轻轻插入7-10cm,若插入受阻,稍停片刻,再继续插入。然后固定肛管,右手打开血管钳,使溶液缓缓流入,并观察反应。如溶液流入受阻,可移动或挤压肛管,检查有无粪块阻塞。如患者有便意,嘱其做深呼吸,同时适当调低灌肠袋,减慢流速。

6.待溶液将要灌完时,夹紧肛管,轻轻拔出肛管放入弯盘内。擦净肛门,嘱患者平卧,尽可能忍耐10min后再排便,以利于粪便软化。对不能下床者,应给予协助。

7.帮助患者穿裤子,整理床单位。协助患者取舒适卧位。

8.打开门窗,撤去屏风,分类整理用物,洗手并记录灌肠结果。

目的:1.为手术,分娩或者检查的患者进行肠道准备;2.刺激患者肠蠕动,软化粪便,解除便秘,排除肠内积气,减轻腹胀;3.稀释和清除肠道内有害物质,减轻中毒;4.灌入低温液体,为高热患者降温。

注意事项:1.注意患者保暖,防止受凉;2.掌握好灌肠液的量,温度,浓度,流速和压力;3.对急腹症,妊娠早期,消化道出血的患者禁止灌肠;肝性脑病患者禁止肥皂水灌肠;伤寒患者灌肠量不能超过500ml,液面距肛门不得超过30cm;4.对患者进行降温灌肠,灌肠后保留30min后再排便,排便后30min测体温。
