私人教练合同条款

           Terms and Conditions of Personal Trainer Contract

健康声明：请您在进行运动之前就您的身体健康情况咨询医生。您理解认同，无论本合同其他条款如果规定，本合同项下的私人教练读完不包括任何诊断，检查或治疗疾病的服务，您同意并承诺在身体有任何症状，包括但不限于外伤、擦伤、疼痛、感染、疾病或虚弱等，且以上症状可能对自己或其他人的健康或安全造成威胁的情况下，不使用俱乐部设备和器械，不接受私人教练服务。我们建议您咨询医生以检查并确保您在使用俱乐部设备和器械时或接受私人服务前，您的身体不存在上述症状，或如果存在上述，该等症状不会对自身或他人的健康安全造成威胁。

Health Statement: please consult a doctor before you start Personal Training Services. You understand and agree, notwithstanding any other terms in this agreement; the Personal Trainer service under this Contract does not include diagnostic ,check, or therapy services, You agree and warrant that you shall not use the Club equipment and facilities nor   personal training service when you have any physical condition, including, but not limited to, exterior wound, scratch, pain, infection, felling sick or weak and the above condition may endanger yours or other people’s health or safety. You are advised to consult a doctor And ensure the above condition no longer exist before you use the Club equipment, programs or facility or use personal training service, or ensure that the above conditions does not endanger yours or other people’s health and safety.

2合同生效：本合同尽在俱乐部确认收到本合同约定款项并由双方签字盖章后生效。

Effectiveness of Contract: The contract becomes effective upon being signed and chopped by both parties, and receipt of payment by Company of the amount set forth in this contract.

3私人教练服务：您购买的私人教练课程权益及权利仅仅限您本人使用。私人教练根据您在第一次训练时需要填写的《健康问答和体测》表中的情况和要求为您制定专业健身计划，提供专业健身指导以及健身过程中的运动及健身保护及健身饮食建议从而协助会员达到自己的健身目标。未经许可俱乐部的不允许外部私人教练进入训练。

Personal Training Service: The personal training sessions can only be used by Member under this Contract. The personal Trainer （“PT”）provides customized physical training plan based on the information and requirement Member has provided under the Fitness Assessment Form which Member must submit prior to the first training session, and professional training guidance and assistance during the training secession, as well as the dietary recommendations in order to meet Member’s training goal. No outside personal trainers are allowed in the Club without the Club’s permission.

4附件效力：会员签署提交的《会员私教课程登记表》和《会员均可问答和体测表》均为本合同附件，与本合同具有同等法律效力。

Appendix: The Membership PT Registration Form signed by Member and Fitness Assessment Form submitted by Member shall be attached to this Contract and have equal weight.

5管辖法律：本合同适用中华人民共和国和北京法律，并根据中华人民共和国法律予以解释。

Governing law：This Contract is construed and interpreted under laws of Beijing, People’s Republic of China.
