Self-employed private physicians who charge a fee for each patient’s visit are the foundation of medical practice in the United States．Most physicians have a contract relationship with one or more hospitals in the community．They send their patients to this hospital，which usually charges patients what they use．Some hospitals belong to a city, a state or, in the case of veterans’hospitals，a federal government agency．Others are operated by religious orders or other non-profit groups．

   Some medical doctors are on salary．Salaried physicians may work as hospital staff members，or residents，who are often still in training．They may teach in medical schools，be hired by corporations to care for their workers or work for the federal government’s Public Health Service．

  Physicians are among the best paid professionals in the United states．In the 1980s，it was not uncommon for medical doctors to earn incomes of more than$100，000 a year．Specialists，particularly surgeons，might earn several times that amount．Physicians list many reasons why they deserve to be so well rewarded for their work。One reason is the long and expensive preparation required to become a physician in the United States．Most would-be physicians first attend college for four years，which can cost nearly$20，000 a year at one of the best private institutions．Prospective physicians then attend medical school for four years．Tuition alone can exceed(超过)$10,000 a year．By the time they have obtained their medical degrees，many young physicians are deeply in debt．They still face three to five years of residency in a hospital，the first year as an apprentice (见习)physician．The hours are long and the pay is relatively low．

When I met him, I had a lot of anger inside of me. I’ve lived my whole life in Spanish Harlem, but in my neighborhood, there are shoot-ups all the time. I know kids who have been shot or beaten up. I have friends who ended up in prison. I could have ended up that way, too, but Mr. Clark wouldn’t let that happen.

Mr. Clark worked long hours, making sure I did my work. My grades rose. In fact, the scores of our whole class rose. One day, he took our class to see The Phantom of the Opera, and it was the first time some kids had ever been out of Harlem. Before the show, he treated us to dinner at a restaurant and taught us not to talk with our mouths full. We did not want to let him down.

On graduation day, there were a lot of tears. We didn’t want his class to end. In 2001, he moved to Atlanta, but he always kept in touch. He started giving lectures about education, and wrote a best-selling book based on his classroom rules, The Essential 55. 

In 2003, Mr. Clark took some of us on a trip to South Africa to deliver school supplies and visit orphanages (孤儿院). It was the most amazing experience of my life. It’s now my dream to one day start a group of women’s clubs, helping people from all backgrounds.

    The local university blood center had a blood drive today at the Civic Center auditorium. Almost 300 people showed up, but about 50 were turned away for various medical reasons. Fifty others left because the lines were moving so slowly. The event concluded at 6 pm, three hours after the scheduled close. It was a long day for everyone-administrators, nurses, and donors. But there were plenty of chairs and tables, and many people brought their own books, magazines, or newspapers.

    “I wish I could hug and kiss all the volunteers that are here today,” said Martha, the blood center donor recruiter. “Many donors underestimate the importance of what they’re doing. They think it is no big deal, but it is a big deal. Their blood is actually saving lives, helping other people to live, which will lead to a more harmonious society. We cannot thank them enough for that, nor can the recipients.”

    With the development of the national economy and the growth of population, the city size grows larger and larger, which brings about both benefits and disadvantages. As a result of the city expansion, local people are enjoying the convenience of commercial business and their residential conditions have also improved. More financial investment will pour into the area as well.

    On the other hand, some destruction of farmland is unavoidable when a city expands. Besides forms of pollution done to the land, wildlife, including both plants and animals, is put at risk.

    All these ought to be considered. In my opinion, tearing down unusable buildings can spare many areas inside the city, so it's unnecessary to expand outside. As for the government, careful planning should be done to maintain the natural beauty while at the same time allowing for growth.
